If an AI-influenced outcome is challenged months later, the organization should be able to reconstruct what information was considered, what the machine proposed and who was empowered to change it.
Capture the decision chain
For each significant decision, record the purpose, authorized input fields, policy or rubric version, system or model release identifier, output, human intervention and final disposition. Do not store a prompt dump merely because it is easy. Sensitive attributes and unrelated personal data should be minimized or separated under controlled access.
Distinguish process evidence from correctness
An audit log proves a sequence was recorded, not that the recommendation was accurate or fair. Independent sample review should compare the decision against original records and current eligibility policy. Error analyses should cover subgroups where lawful and ethical to assess, unusual cases and adverse outcomes.
Design a real correction mechanism
A person needs a reachable process to challenge mistaken input facts and seek competent human review. Determine any legally required notices and appeal mechanisms under the actual jurisdiction and application, rather than claiming every automated process has the same rule. Retain correction history and prevent a stale decision from being silently reissued.
Protect the audit trail
Logs can expose personal data, protected client material and business secrets. Apply least-privilege access, integrity controls, retention schedules and separation between developers, operators and reviewers. Ensure incident responders can explain which system versions and policy changes affect a questioned cohort.
Operational test
Take ten previously resolved cases and attempt reconstruction without relying on the original operator’s memory. If the team cannot identify authoritative input, controlling rule and override decision, remediation is needed. NIST AI RMF is voluntary management guidance, not an assurance that recordkeeping alone satisfies local due process law.
