Context Windows Explained: Input, Output, Lost-in-the-Middle and Truncation

A context window is a capacity constraint, not a certificate of understanding. A model advertised with one million input tokens may accept a large document but still fail to locate a critical clause, combine two distant details or respect.

A context window is a capacity constraint, not a certificate of understanding. A model advertised with one million input tokens may accept a large document but still fail to locate a critical clause, combine two distant details or respect a required citation format. Input budget and maximum output are separately enforced and differ by model and deployment. Proper evaluations must distinguish token limit errors from reasoning failures.

Budget all message parts

Count system instructions, tool descriptions, retrieved documents, user messages, intermediate results and the desired answer. Some providers count reasoning tokens and multimodal attachments differently. For a 65K output maximum, a request for 100K output tokens will not succeed regardless of context. For images or video, nominal text-token counts may understate encoded input usage.

The long-context evaluation

Build a test set with facts at different positions in a long document, decoys sharing keywords and questions requiring evidence from two distant places. Keep a gold source-span map. Grade not only the answer but citation location and abstention when evidence is absent. Test at short, medium and near-limit context sizes to expose degradation.

Truncation and retrieval

Some APIs error on oversized input; others offer automatic truncation settings. Silent truncation can remove the very evidence a user needs. Prefer explicit budgeting, chunking and a retrieval layer with document permissions. Where stakes are high, rerank retrieved passages and present them to the model with location labels. Human readers should be able to inspect the primary evidence.

Practical example

A company has 500 contracts, but the user asks for cancellation deadlines in five. Uploading everything into a long prompt is not automatically better than locating the five contracts and relevant clauses. The proper test measures false omissions and wrong cross-contract attribution, then compares workflow time and costs. Do not mix stable and preview model windows when reporting a result.

What to verify before adopting this model or method

Use the exact release ID and API endpoint; confirm your geographic processing location, entitlement tier, model lifecycle, feature flags, context and output limits from the provider console. Run a reproducible evaluation on your own data and retain human oversight for consequential actions. Where a specification, price, commercial license or entitlement is not verified for your deployment, this article deliberately leaves it unasserted instead of filling a plausible number.

Sources & Further Reading

Source documentation establishes vendor-published facts, not independently measured model quality, tenant access, contractual rights or current prices. Use a dated pricing ledger and an account-specific entitlement check for deployment decisions.
